There is a cost associated with the release of some reports. To facilitate requests for reports and records, below

FIRE & EMS REPORTS

Reports are available at the fire station. Fire Headquarters located at 77 South Fourth Street in McConnelsville,
Oh. 43756 is the only authorized location to perform this function.
To request a Fire or Emergency Medical report, you may call 740-962-3444, or fax your request to 740-962-4798, or submit an e-mail to: mm.fire@yahoo.com
Request for reports must be made in writing. Any form of written correspondence (letter, subpoena, etc.) is sufficient as long as we have patients original signature for EMS runs request.
